You have a choice where to go for physical therapy. #Choose Sport and Spine.

Specializing in Orthopedics and Sports Medicine

Founded in 1988, Sport & Spine Physical Therapy is an outpatient physical therapy and rehabilitation practice that specializes in orthopedic and sports injuries.

We are dedicated to providing the highest quality of care in a friendly and encouraging environment. In each of our clinics we offer a team approach so you can receive the best treatment possible. Our team-mates consist of licensed Physical & Occupational Therapists, Certified Athletic Trainers and Physical Therapy Assistants.

We understand many patients have a need for braces and splints. Therefore, we have a DME license. This allows us to create custom or use off-the-shelf braces and foot orthotics for a complete recovery.

We enjoy being an active part in our community. Sport & Spine has provided physical therapy services to local high schools for the past 20 years. We are also associated with other physical therapy practices in the surrounding areas including: Merrill, Auburndale, Medford and Wittenberg.

We accept most insurance including Medicare and Worker’s Comp.

FAQ

For Additional Questions Please Contact Us

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.

We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physician’s referral for therapy to be covered. We also offer treatments, services or products that do NOT require a referral and are available on a self-pay basis.

We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.

As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.

Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– Referral slip from your medical doctor
– Your medical insurance card
– Your ID
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.

Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.

We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.

Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.

Please feel free to call your physical therapist if you ever have any questions regarding your treatment or your home exercises.

The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.

This is my second “trip” and I’d make Sport and Spine my physical therapy clinic of choice. I have referred patients to Edgar due to the phenomenal care and staff.

Very friendly staff – they care about the person. They work hard to ensure you are “fixed” and provide everything you need to go home and stay healthy.

The staff was extremely knowledgeable, very courteous, and they all had a great sense of humor. The receptionist always made you feel like she was welcoming you home when you walked in the door. Therapy was made to fun. I didn’t want my time to end!

KP

In March I had a total knee replacement. Jeremy and Deb work as a great team. They kept me motivated so that I had full range of motion in 6 weeks. I actually looked forward to going to therapy because I felt a tremendous improvement after every session. I highly recommend the team of Jeremy & Deb to anyone needing therapy.

TR, Edgar Location

Eight months ago, I was in a work accident and broke and degloved my leg. With this, I lost a lot of tissue and all my muscle strength. Before the accident, I was a runner and I told my therapist I wanted to at least bike again. When June came along I was well over biking and made a goal to run the Athens 5K. As the summer moved on I started doing all the things I love to do like, tubing, water skiing, and bowling. I got to enjoy my summer more then what I thought I would back when I was in the hospital. By August, I was able to go back being on my Volunteer Fire Department in Athens. At the Athens 5K, I did way over my goal at 40 minutes and finished in 30 minutes and 15 seconds. I'm not done healing yet and can't wait to see how I will keep improving on my running.

Picked you by reference. However, you did such a wonderful job. Will recommend you to everybody
